Helpful Hints When
Ordering Records
Thank you for giving SRS the
opportunity to be a solution for your record retrieval needs. We
believe that quality, blended with excellent service, is the
foundation for successful business and trust the following suggestions
will help accelerate the process of retrieving records.
- Include the dates of service for
each order requested.
- Include as much personal
information for the patient, including SSN and DOB.
- Include a medical record number or
case number if known (Primarily with large HMO’s etc).
- Provide the complete and most
specific name for the provider, (all providers not listed in our
database will be verified and then stored for future searches).
- Provide the correct city and phone
number for the provider if applicable. (if unknown, please leave
blank)
- When ordering pharmacy records,
only a request for billing record is needed. The reason for this is
that when the records are received, they are in list format of what
was prescribed and the amount that was billed.
- Use the instructions box to
provide any additional information, including patient address for
the last three years.
- Having a secondary contact
available will expedite the retrieval process, if the primary
requestor is not available to resolve outstanding questions.
- Include your e-mail address when
requesting records & check box on order form if you desire automatic
notification via e-mail.
- If possible, list SRS’s name on
each authorization to ensure that the provider & client are aware
that your company has outsourced record retrieval.
- If the provider sends the
requested records directly to your site, please notify SRS with the
amount of pages received, so we will be able to close the order on
our end.
GLOSSARY
Driving History/Traffic Accident
Search
Results will show accidents, violations, actions, drunken driving
arrests, tickets, vehicles, etc. These searches have revealed that
subjects may have been injured in a traffic accident immediately prior
to reporting an accident at the work place.
Motor and Recreational Vehicle’s Ownership
This search will provide you with all motor vehicles, recreational
vehicles and trailers registered to the subject of your investigation.
This may show that your subject possesses vehicles or “toys” that
don’t match his/her economic profile.
Criminal History Search-Federal, Statewide or by County
Research conducted through criminal history records has shown that
some subjects have been involved in criminal activity prior to or
immediately after their injury, and in some cases they have been found
to have been injured as a result of their criminal activity. Records
are available from any county in the United States and include a
subject's criminal record dating back seven years (longer upon
request).
Civil Court Records-Federal, State, County
Legal actions provide detailed, documented reports of the subject’s
personal history, background and financial relationships. Records are
available from any county in the United States.
Occupational Licenses
Many industries or trades do not require state licenses. However, most
counties and municipalities do require that any person conducting
business in their jurisdiction obtains an occupational license. You
may find that your subject has obtained such as license, either under
his name or a fictitious name.
Corporate Search
Lists any and all corporations registered to the subject. Some people
have incorporated their own businesses and show no income under their
names. However, they may be conducting the business themselves and
paying a salary to their spouses or children.
Judgments
Include lower court civil judgments such as municipal and small claims
actions not already included in a civil court search..
Consumer Affairs
Updated monthly, California Professional License Index searches up to
38 occupational license types to provide the name and address of the
licensee, license number, date of original issuance, and date of
expiration. Status of license is also provided.
WCAB Background Workers' Compensation Appeals Board (WCAB)
The WCAB records cover any work-related injuries that are taken to
court. Records are available from 1992 to present, and in many cases
go back farther than that.
