CONTESTABLE DEATH CLAIMS
Involves investigation designed to develop information relative to the insurability of the deceased at the time of effective date of coverage. Arrange interviews and signed statements concerning health history, medical providers and develop any omitted factors on application prior to issuance or delivery of policy. Areas of investigation include but not limited to: details of death as known to informants, medical, financial, occupational habits, and any other outside interests.


ACCIDENTAL DEATH CLAIMS
Designed to determine whether the death resulted from injuries sustained in an accident, altercation, sickness, and/or whether death occurred while in the act of committing a felony. Our team will secure official reports, conduct interviews with those necessary, to get full details of events. Media reports, statements of available witnesses – when unable to obtain from the authorities, Coroner’s reports with toxicology results are all a part of our investigation.


HOMICIDE / SUICIDE CLAIMS

CONTESTABLE DISABILITY CLAIMS
Investigation to determine information about the insurability of the policyholder at the time, not the effective date, of coverage. Making announced and/or unannounced interviews of claimant to determine activities, continuity of disability, progress, and delving into circumstances surrounding claimed disability with emphasis on any pre-existing or omitted factors on application for coverage. SRS offers you the choice of two types of interviews—a Written/Telephonic Statement or a Field Interview. Written or Telephonic Statements are conducted by our in-house investigative team or field representative, and include an interview of the subject(s). This interview includes a written or recorded statement covering the nature and cause of injury/loss and all relevant issues pertaining to the claim. Our Field Interview involves a personal face-to-face interview with the subject. You receive a professional, detailed report that includes the representative’s comments and signed statement.

ACTIVITY CHECKS

An activity check provides valuable information of the subject's current lifestyle. Its purpose is to provide a synopsis of the subject's physical and mental state, as well as information regarding possible subsequent or current employment.

SRS’s Activity Check includes discreet inquiries that lend insight into the claimant’s present level of physical capabilities, activities, hobbies and possible employment status. Through this investigative process, a description of the claimant’s residence, neighborhood and surroundings is determined and additional relevant information pertaining to the claimant is provided. We can help you uncover important information and records from the past that may offer further insight into your subject’s current activities.

Many cases are most effectively investigated by first initiating an activity check. An activity or lifestyle check can provide valuable information before considering surveillance or other forms of investigation. Activity checks can be preformed in the field or via telephone.

BACKGROUND ACTIVITIES INVESTIGATION
Clients involved with new claims or long-standing litigated files typically request this service. An experienced SRS investigator will personally visit a subject's neighborhood and discreetly interview residential sources to develop background information regarding the subject's activities, employment, income and health. We primarily concentrate on developing information regarding the subject's current employment status and/or pre-existing or subsequent injury information, health history, patterns of arrivals and departures from home, involvement in home remodeling and maintenance, and participation in sports and/or memberships in social groups and clubs. If required, or at the client's request, we will obtain a criminal history record, MVR, workers compensation claim history, This is our most comprehensive and detailed background investigation.

BASIC BACKGROUND INVESTIGATION
Provides information useful to determine the general character and trustworthiness of an individual. This investigation will normally ascertain the subject's true identity, residence address, business ownership interests, employer or other income sources. In addition, a thorough search of the subject's civil and criminal background history may be undertaken within the region the subject currently resides or in counties, states or countries of previous residence or travel. The investigation may provide the subject's driving history and personal financial profile including ownership interests such as real property or motor vehicles and liabilities including bankruptcies, civil judgments, notices of default or foreclosure, and state or federal tax liens. A personal background investigation may be fundamental or complex depending upon the subject individual and matters at issue.

BUSINESS BACKGROUND INVESTIGATION
Provides information useful to determine the structure and condition of any business entity, be it a sole proprietorship or multinational corporation. Working with minimal information, SRS will determine the correct business name, size and type, ownership and key executives, facility locations, holdings and interests, business activities and practices; as well as civil litigation, criminal conviction and regulatory compliance histories. Additionally, a business background investigation can identify assets and liabilities; the background of principal officers and directors; or company suppliers, vendors, employees and customers.
